We use and recommending using RBL -- real time black hole lists.
Utilizing them has cut down spam from 85% on bad days to 95% on good days.
I would recommend looking into RBL's as well as making sure you are using
the most up-to-date version of SpamAssassin (2.61).

- I have procmail rules and Spam Assassin (set at 4.4) filtering.
- I have over 1500 blackmail listings in procmail.
