Solved

This caused problems:-
> :0:sendmail.lock
> * ... conditions here
> :/usr/sbin/sendmail

Like this...
> procmail: Skipped "/usr/sbin/sendmail"
> procmail: Skipped "H:sendmail.lock"

I had...
> :0:
> * conditions
> !nouser

I removed the ":" after :0 and it now works fine.

So I should start every recipe with :0:sendmail.lock and end it with 
:/usr/sbin/sendmail
