Ghislain,
Thank you for all your help. We have again figured this out. It
appears the client has changed the passwords on their computers and
didn't tell us. This was the result of a multiple issues happening at
the same time.
1. They were using the login username of "username@xxxxxxxxxx"
2. They changed their passwords.
I was finally able to duplicate the issue by logging in using an
incorrect password.
The client is now happy that we have solved the issues.

do you have the same issue with any other client i mean thunderbird or
eudora ?
is outlook patched at the last version ?
is the mailbox empty or not ? if you empty it does it work anew or do
the error continue?
do you saslauthd daemon is running ?
seems their connexion is broken in the middle of the process. You have
to see if this is an outlook trouble only or a server trouble (can be
both also...)
at last check the permition on the mail files.
