I haven't had any false positive complaints so far <fingers crossed>. But since using the blacklists, two important and useful events have happened -

1) Client became aware of their own vulnerability - I had one client running their own mail server that had been hijacked by spammers. They were unaware of the activity until I turned on some DNSBLs. Then they found out the hard way. Took a while to clean up the mess, but it was a valuable lesson and there's one less spambot out there now.

2) Client went after their ISP's for abuse - After I started using DNSBL's one client discovered that his ISP was inadvertently hosting a spammer and had been blacklisted. They complained to the provider, got the account cancelled and the block removed. Again, it took time and patience, but there's one less spam source now.
