Yes, the freshclam should run by default at least twice a day (I
think), but it may not be indicating errors in the cron that it might
otherwise do at the command-prompt, but if you're running that from a
shell as well, we can assume you're up to date.
The second thing to try is resending a virus that got through and see
why it's not getting caught. ClamAV does a fair job, but I don't think
it's perfect. It is definitely not a substitute for a Windows virus
scanner that runs locally on the client, but it can be a good first
line of defense.
