In theory, huh :)
Unfortunately, SA on my vps1 can't handle the 15 MB mbox file, so it
says "out of memory" I have a feeling that its reading it all as one
email, rather than separate messages. virtual spamassasin -h gives
me directions for reading a message, but not an mbox.
I copied the file to my vps2 account and it ran a little longer, but
same out of memory error. :(
Any other suggestions, other than bouncing the emails back to himself
via mutt (for 4000 messages!)

try making a copy of the mailbox and run
spamassassin < name_of-your_copy >new_name and see what happens.
In theory it would create this new box adding the tags
