I'm working with Alvin Tan on this one too and thought we had this
generally licked. Tell me, is the account where this happened a VPS2-A
(or basic)? Alvin's problems have generally come when the server was
out of processes (it's a VPS2-A).
I've not seen the bug occur in production for higher-end accounts
where more processes are available. There used to be a race condition
that would happen on *any* account regardless of processes. That
condition has been eliminated with careful locking and access
serialization.
